Paranoia comes back while being treated

Hi I have been under medication on lithium and risperadone for over a year now. I have been doing fine except if something comes up that stresses me out then I can get the paranoia back and I just waves of cloudiness and feeling flustered. Does this mean my medication is not working and I need to up my dosage?

These are all new symptoms since being diagnosed with bipolar.
